Horizon Celebrates 25 years!
July 1, 2015 marks the twenty-five-year anniversary of Horizon Home Care & Hospice.

After speaking with Kris, a few clear reasons surfaced as to why she believes Horizon has continued to grow into the premiere agency it is today.

A Brief History: 25 years
Horizon was founded in 1990 under the name "Horizon Home Care & Staffing, Inc." It was a joint venture between four Milwaukee area hospitals (Columbia, Froedtert Lutheran Memorial, Community Memorial, and St. Mary’s hospitals) to merge their home care, hospice, private duty, meals-on-wheels, and non-medical care activities into one organization.
In 1992 Horizon was granted it's not-for-profit status and was renamed Horizon Home Care & Hospice Inc.
In 1995 Horizon earned accreditation from The Joint Commission, which it still holds today.
In 2010 Horizon opened an inpatient hospice unit in Mequon within Columbia St. Mary’s Hospital to serve its growing census of hospice patients.
