Services

Horizon Hospice offers hospice care in the home, nursing homes, assisted living, and at our two inpatient hospices.

Horizon is a not-for-profited, Joint Commission Accredited, licensed and certified Hospice, one of the only in the area, an important distinction when choosing care.
